What is a 7d Physical?
A 7D Physical is a state-required medical examination for drivers of school buses, vans, and other pupil transportation vehicles in Massachusetts. It ensures that drivers meet medical and safety standards set forth by the Massachusetts Registry of Motor Vehicles (RMV)
At Rapid DOT we have a simple 3-step process to keep your appointment as hassle-free as possible. The entire 7d exam (at our office) lasts less than 30 minutes from start to finish.
Step #1: The Federal Paperwork
You’ll begin by completing the first section of the Medical Form for a School Pupil (7D) Driver Certificate or a School Bus Driver Certificate This should take no more than five minutes.
On this form you’ll be asked things like…
> Have you ever had any surgeries?
>Are you currently taking any medications?
>Do you have any cardiovascular problems, kidney issues, hypertension, diabetes, sleep apnea, etc…?
Step #2: The 7d Medical Exam
The exam portion is where the doctor will review and clarify any of your answers to the paperwork portion that you completed. Then, he will conduct a brief basic physical exam.
The exam lasts less than 20 minutes and includes things like..
>Urinalysis: You will be asked to provide a urine sample. THIS IS NOT A DRUG TEST. Instead it’s a screen for uncontrolled diabetes and any underlying kidney issues.
>A check of vitals including blood pressure, vision and hearing.
>General Systems Review: A quick scan of your cardiovascular, pulmonary and neuromusculoskeletal health.
Step #3: The 7d Driver Certificate
Once your exam is completed and meets the requirements of the Massachusetts RMV, you will receive your signed and completed medical form to obtain and renew your 7d certificate. In addition, you will also receive a complimentary DOT medical card to use for other driving opportunities.
The DOT card will have all the information you need for the following licenses:
>Commercial driver’s license (CDL) for any state.
>Hoisting/Hydraulics Engineer License

No. The urinalysis that is a part of the DOT exam is strictly a screen for any sugar, blood of protein in one’s urine. The urinalysis is used to identify any potential uncontrolled diabetes or kidney conditions that may be underlying and affect the driver’s safety.

No. Insurance will not cover a DOT physical. Some employers may cover the cost of the exam, but it’s best to check first.

The RMV requires a new 7d certificate every year.

Yes – certain medical conditions such as uncontrolled hypertension, uncontrolled sleep apnea, uncontrolled diabetes and certain cardiovascular conditons may prevent a driver from passing the 7d physical.
In most cases, these conditions can be controlled with medications. The Mass RMV has specific required forms for 7d drivers with diabetes, carvascular conditions and pulmonary conditions. You can access those documents in the “forms” dropdown below.
