When you arrive for your appointment, you'll complete the necessary paperwork (if you haven't already done so beforehand). Then, you'll be taken into the exam room where you'll meet with our National Registry Certified Medical Examiner.
The DOT physical exam will include a review of your health history and medications as well as a hands-on medical evaluation. The medical exam portion will evaluate your vital signs, vision, hearing, heart, lungs, back/spine, arms/extremities and overall well-being.
Upon successful completion of the physical, you will receive your DOT medical card.

A DOT medical card is required for any commercial driver who operates a vehicle with a gross weight of 10,000 pounds or more. This applies to all commercial driver's license (CDL) holders as well as non-CDL holders including hoisting/hydraulic operators, box truck drivers, commercial bus drivers, limousine drivers, and others.
Drivers of vehicles designed to transport more than 15 people (or 8 people when there is compensation) or hazardous material are also required to maintain a valid DOT medical card.

In Swampscott (and all of Massachusetts), all DOT exams must be performed by a provider certified through and listed on the National Registry of Certified Medical Examiners. A full bladder (for the basic urinalysis)
Current medication list
Most recent Ha1C (if a diabetic)
Form MCSA-5870 if an insulin-dependent diabetic
CPAP compliance log (last 90 days, if diagnosed with sleep apnea)
Recent tests results and clearance letter from your cardiologist and/or neurologist (if you have a history of heart disease, stroke, seizures or other neurological disorders),
Last 12 months of INR readings (if you are prescribed a blood thinner).

If you hold a commercial driver’s license in Swampscott, MA you are also required to submit a copy of your valid DOT medical card to the MA RMV. This process is commonly referred to as self-certification and can be completed online or via mail.
